Appointments

To schedule an appointment, please call (605) 341-2424 or (800) 253-5876. We encourage you to arrive 30 minutes before your scheduled appointment so we may review your medical history and new patient registration forms with you. If you need help completing the forms, or if you are uncertain about the location of our office, please call us prior to your appointment at (605) 341-2424.

Schedule an Appointment

During your initial visit, our doctors will review your medical history and evaluate the condition for which you were referred to us. The doctor may be assisted in your examination by one of our physician assistants. Depending on your condition, your first visit may take an hour or more, so please allow plenty of time in your schedule.

What to Bring to Your Appointment

Please help us provide you with personalized care and reduce your waiting time by bringing the following items to your appointment. If you do not provide the information requested, we may need to reschedule your appointment.

• X-ray films, medical records, and/or any other test results relating to your present condition. If you are taking medications, please bring your medicine or the information on the label.
• Please bring your completed medical history and new patient registration form. These forms are available by calling our office or may be printed from this website. We require both forms to be fully completed at the time of your appointment.
• Evidence of insurance. Please bring information (i.e. insurance card) regarding your participation in a health plan or provide confirmation of workers' compensation coverage. Your insurance company or health plan may require you to pre-authorize your care.

Rescheduling Appointments

If you are unable to come to a scheduled appointment, please notify us at least 24 hours in advance. If you will be late for your appointment, please also call us in advance. This courtesy allows us to be of service to other patients. We can reschedule your appointment at the earliest available date. If you miss your appointment without notifying us of cancellation, you may lose priority when rescheduling. You might also be charged for your missed appointment.
