After hours, calls are received by an answering service that can page our on-call physician as needed. In case of an emergency, call the nearest Emergency Room or dial 911.

It contains details about payment, fees, insurance, workers' compensation, Medicare, Medicaid, Title 19, motorized vehicle accidents and personal injury claims. Fees

Our fees reflect the level of care provided and are within the customary range for this area.

Payment

Payment for services is required on the date of service unless other arrangements have been made prior to the appointment. We accept cash, checks and credit cards. You are responsible for your entire medical bill regardless of your insurance coverage. We strongly encourage you to be aware of their guidelines for payment and make plans accordingly.

Confidentiality

Your medical records are strictly confidential. We will not give information regarding your condition to your employer, friends or relatives without your permission. As required by law, we release medical information for workers' compensation, personal injury claims, assault, etc.

Insurance/Coverage

As a courtesy to you, we will file your insurance claims. If your insurance company sends us payment, a refund check will be sent to you. We participate with many different health insurance programs and follow guidelines set by these plans. The programs we participate in are listed at the bottom of this page.

If you have more than one insurance company, please identify the name of the carrier on the New Patient Registration form.

Details concerning your coverage, deductibles, co-payments, secondary insurance and other such information should be discussed with your insurance carrier. We will not become involved in disputes between you and your insurance company.

Evidence of Insurance/Preauthorization

Your insurance company or health care plan may require you to preauthorize your care. Failure to preauthorize could result in a reduction of payment by your insurance company or health care plan. If you are scheduled for surgery, our staff will review costs, preauthorization, admissions and other relevant information with you.

Worker's Compensation

Workers' compensation insurance is different from the health insurance provided by an employer. Prior to your appointment, you must have your workers' compensation insurance carrier or employer call us to authorize your care or you will be required to pay at the time of service.

Medicare

  • We accept Medicare assignment of benefits.
  • You must pay your deductible and co-insurance amounts at the time of service.
  • We will file a claim with Medicare and payment will come to us.
  • If you have supplemental insurance, we will file a claim after Medicare has paid.

Medicaid

  • Referral from your primary care provider is required.
  • You must bring your purple referral card and current month's identification card to your appointment.
  • You will be required to pay a co-payment amount as indicated on your identification card.

Motor Vehicle Accident and/or Personal Injury

  • You are responsible for payment of fees for your care even though other liability may be involved.
  • Motor vehicle insurance medical coverage is usually limited to a certain dollar amount. Your personal health insurance may pay for care after that amount has been exhausted.
  • We require private health insurance and motor vehicle insurance information. Please notify us in advance if legal action is involved.

Indian Health Services

You must have authorization from Contract Health Services at your Indian Health Services Hospital prior to your appointment. Please have Contract Health Services call us with your purchase order number before your visit.
